Welcome to the Transcript/Immunization Request Form.

Transcripts will take up to 10-14 business days to be processed. Transcripts cannot be emailed.

You may do one transaction after another if you need more than one form. Each transcript request is $10.00. Each immunization form request is $10.00. Please complete the form below with all of the information. Be specific as to your choice of transcripts, immunization, or both. Please let us know where you will want the form to be sent.

SCA only has immunization records for the Classes of 2012 through 2022.

Transcripts will not be processed during SCA's Christmas Break.
